A Place 4 Me — $20,000
Help ensure vulnerable young adults have access to safe housing navigation and supportive services
Funding will support navigation services at a new Youth Drop-In Center located within the former Joseph’s Home building on the St. Vincent Charity Health & Healing Hub campus in Cleveland’s Central neighborhood, providing critical support to young adults experiencing homelessness or housing instability.
Care Alliance Health Center – Expanded Hours Initiative — $150,000 over two years
Increase access to affordable, convenient health care for residents
Funding will expand Saturday clinical hours at Care Alliance’s Central neighborhood location, increasing access to primary care, behavioral health, pharmacy services, patient navigation and case management for residents who cannot access care during traditional weekday hours.
Care Alliance Health Center – Community Health Worker Initiative — $163,000
Strengthen educational success by helping children and families overcome barriers that affect health, attendance and academic achievement
Funding will place two community health workers inside Marion-Sterling School to address chronic absenteeism by connecting students and families with health care, social services and individualized support.
Cleveland Leadership Center — $55,000
Strengthen collaboration, helping organizations work together more effectively to serve neighborhood residents
Funding will support a first-of-its-kind community asset mapping initiative and collaborative think tank series designed to identify service gaps, strengthen nonprofit partnerships and improve coordination among organizations serving the Central neighborhood.
Collaborative to End Human Trafficking — $30,000
Protect vulnerable residents through education, prevention and coordinated community response
Funding supports trauma-informed prevention, education, professional training and survivor-informed outreach to identify and reduce human trafficking throughout the Central neighborhood.
Environmental Health Watch — $10,000
Promote healthier lifestyles while celebrating community pride and neighborhood engagement
Funding supports health and wellness programmatic components of Fresh Fest Cleveland, a neighborhood celebration promoting healthy food access, entrepreneurship, arts and wellness in the Central-Kinsman community.
Foluké Cultural Arts Center — $75,000
Expand opportunities for residents of all ages to improve health, reduce isolation and build stronger community connections through the arts
Funding will support multigenerational arts programming hosted at Friendly Inn Settlement while also strengthening the organization's long-term sustainability through strategic planning and organizational development.
Friendly Inn Settlement — $88,210
Combat senior isolation while improving nutrition, wellness and quality of life for neighborhood older adults
Funding launches the Golden Connections Senior Program, providing meals, wellness programming, social engagement and resource navigation for older adults in Cleveland’s Central neighborhood.
Goodwill Industries of Greater Cleveland and East Central Ohio — $114,000
Accelerate transformational redevelopment that will expand economic opportunity, food access and community resources
Funding will support a capital campaign consultant for the Cleveland Opportunity Center, helping advance fundraising for the future Opportunity Center and grocery store located on the former St. Vincent Charity Hospital campus.
Joseph & Mary's Home, a ministry of the Sisters of Charity Health System — $35,000
Ensure medically vulnerable individuals receive the care and stability needed to recover and transition toward permanent housing
Funding supports ongoing operations for the organization’s medical respite program, providing compassionate care for adults experiencing homelessness who need a safe place to recover following hospitalization.
Light of Hearts Villa, a ministry of the Sisters of Charity Health System — $50,000
Ensure vulnerable seniors continue receiving compassionate care with dignity
Funding supports the Sister Helen T. Scasny, SC Benevolent Fund, helping older adults remain in assisted living or memory care when personal financial resources have been exhausted.
Regina Health Center, a ministry of the Sisters of Charity Health System — $20,000
Nurture the spiritual, emotional and holistic well-being of older adults in the Sisters of Charity tradition
Funding supports pastoral care programming that serves residents through spiritual care, counseling and faith-based support services.
Seeds of Literacy — $10,000
Create pathways to education, employment and greater economic mobility for neighborhood residents
Funding supports adult basic education, literacy instruction and General Educational Development (GED) preparation for adults living in Cleveland’s Central neighborhood.
